This document is an official offer (public offer) of Dentoria Family Smile Clinic (hereinafter — the Provider) to enter into a contract for the provision of dental services with any individual (hereinafter — the Patient).

Subject of the contract: The Provider undertakes to provide dental services, and the Patient undertakes to pay for them according to the current price list.

Procedure for providing services: Services are provided by appointment. The Patient undertakes to arrive on time for the appointment and provide accurate information about their health status.

Cost and payment: The cost of services is determined based on the approved price list. Payment is made on the day the service is provided.

Liability of the parties: The Provider is responsible for the quality of services in accordance with the law. The Patient is responsible for following the doctor's instructions.

Acceptance of the offer: By paying for services or signing an informed consent to medical intervention, the Patient confirms their agreement with the terms of this offer.
